The following plans are currently available from the AMA Plans Service at the prices indicated. Please see the order form to place your order or call the Plans Service at (765) 287-1256, ext. 507. For best handling, please do not include payment for any other service with your plans order. 

Please allow at least one to three weeks for delivery within the United States—longer for overseas. All plans are sent by First Class mail.
